Why we started Leland

We went looking for the person whose job was the whole story. There wasn’t one.

The average older adult on Medicare sees seven doctors across four practices. Each one sees a slice of the picture. Nobody sees the whole thing. We started Leland because we went looking for the one person, anywhere in that patient's care, whose job it was to hold the whole story together, and we could not find them.

No one was accountable for the whole. Now Leland is.

So we are building that person's job, and the system that makes it possible to do. We pull a senior's scattered health and life information into one living record. An intelligence layer reads the whole picture and spots what matters: the missed screening, the medications that conflict, the risk that becomes an ER visit if nobody moves. And AI agents do the coordination work that never makes it onto a chart, scheduling the follow-up after a hospital stay, arranging the ride, chasing the referral, reconciling the medication list.

And at the center of all of it sits the advocate: a trusted human who builds a deep relationship with the senior and their family, and who is the warm interface to the intelligent system working behind the scenes. The family never has to run the machinery. They talk to one person who knows the whole story and stays accountable for what happens next, not just for what happened at the last visit.

Older adults managing several conditions at once are the ones the system currently serves worst. Closing that gap is the whole reason Leland exists.

It works in four layers. One complete record of a senior's health, the thing nobody else had ever built. Intelligence that reads it and flags what needs attention before it becomes a crisis. Agents that handle the follow-ups, the scheduling, and the paperwork. And on top of all three, the advocate: one trusted human who uses them to care for the whole person, and answers to the family for all of it.
