About Leland Care
The typical US senior sees seven physicians across four practices plus insurers, pharmacies, family members, and facility staff. No one has the full picture. Fragmented data in incompatible systems leaves seniors and families managing chaos instead of focusing on living well.
Leland is building the first unified system of record for aging to help seniors live longer, healthier, and happier lives. With one complete record of a senior's health and life, AI can intelligently analyze the full picture to surface risks, gaps, and opportunities, while the senior's trusted human advocate guides the next best actions and removes friction so families can actually follow through. This turns fragmented information into proactive, coordinated support that extends healthy, independent years.
We have a multi-year funding runway and are launching pilots in 2026 with providers and already in active conversations with major US health systems. The company is led by a Stanford GSB founder who built and led Sword Health’s behavioral health division and co-founded Meomind.
About the role
Leland patient advocates are the people our seniors and families call by name. As a full-time advocate you carry a small group of families remotely, by phone and video: after-hospital follow-ups, referrals, medication lists across pharmacies, insurance denials and appeals, joining appointments by phone, and keeping the whole family updated. You work inside a physician-led practice with the living record behind you, so you always have the full picture and the routine work is drafted and tracked for you.
What you'll do
- Carry your families end to end: Own the follow-ups, referrals, medication lists, denials, and appointments for the seniors and families assigned to you. You are the one constant in their care web.
- Close the loop on every call: Get the answer from the doctor's office, pharmacy, or insurer, log it in the record, and tell the family what happens next.
Who we're looking for
- Current, unrestricted RN license in good standing. A multistate (compact) license is a plus.
- Experience working with patients or older adults: patient advocacy, care coordination, nursing, or case management.
- Warmth on the phone. Families should feel better the moment they hear your voice.
- Fierce follow-through. Nothing you own falls through the cracks, ever.
- Comfort working alongside AI tools that draft and track the routine work. You check everything.
- A quiet place to work and a reliable internet connection, anywhere in the US.
- Spanish is a big plus.
